Better replica set connection error handling
Added MongoCursor::batchSize()
Added mongo.allow_empty_keys option
Added unique codes for all MongoExceptions and MongoCursorExceptions
Added MongoId::getPID() and MongoId::getInc() methods
Added $id field to MongoId so that json_encode turns it into {$id:...}
